Minor Waiver and Release
- Annual Waiver for NASA Officials
NASA has an annual waiver program available for NASA Officials only. This program will serve as your NASA waiver for the year but it may not suffice for the individual tracks so you may have to sign another track specific waiver at the gate. You will be issued a special credential that will attach to your Official ID card. To participate in this program, please download this form and have your signature notarized. If you already have a NASA Official ID Card that has your photo on it please send it along with the form to the National Office at P.O. Box 21555, Richmond, CA 94820-1555. If you do not have an Official ID Card with your photo on it or would like a new one, please send the notarized form and photo through the mail to the address above. You can also e-mail a headshot photo to dave@drivenasa.com.
